Prior to your first appointment, we will need your medical information. We will send you a link by text or email please fill out that encrypted form which will be automatically sent to our office or if you have any accessibility needs or questions about the forms consent, please arrive 20 minutes early to your first appointment, and we would be happy to help you complete the forms in person.
AFTER HOURS EMERGENCIES
If you are a patient of Dentistry at the Inn on Lake Bonavista and you have a dental emergency, please call the office at 403-271-0106 to retrieve the emergency number.
